Membership Forms
New and renewal memberships must be processed in person at Cottonwood Heights Recreation Center.
All change requests are not guaranteed and are subject to approval.
Quarterly & Annual Memberhship Forms
Holds for Memberships are only approved for Military or Medical reasons. Note from Physician required.

Upgrades to yearly memberships are allowed only for members with at least 6 months remaining on their current membership. Members will pay the difference in membership cost plus a $20 processing fee. All upgrades must be completed in person with a Customer Service Representative at Cottonwood Heights Recreation Center.
Quarterly memberships are not eligible for change at any time.
Monthly Membership Forms
ONLY those who have met the terms of their contract will be approved for cancellation.
